Anti-Coagulant
The Anticoagulant Therapy Management Service is designed to deliver anticoagulant therapy management for patients aged 16 and over.
The service provides clinically effective anticoagulation management, minimising risks and ensuring patients maintain a stable INR (International Normalised Ratio).
The service is delivered at 9 of our member practices (including home visits where necessary) offering care closer to home.
