The diabetes integrated model of care service supports high quality treatment and prevention of diabetes by enabling a seamless pathway for adults living with diabetes, providing a strong focus on prevention and education.
Working in unison, both primary and secondary care clinicians collaborate to ensure patients’ needs are met, offering a holistic approach through personal care planning and empowerment of patients to effectively self-manage their care.
SDH work closely with lead diabetic nurses and GP’s, supporting locality meetings and training events to ensure collaborative working and equity of care to a high standard.
Secondary care consultants and specialist nurses attend GP practices at regular intervals to support the care of more complex patients and help further education in primary care.
