VAWAS
The Vulnerable Adults Wrap Around Service (VAWAS) provides integrated, community-based care aimed at preventing unnecessary hospital admissions for vulnerable individuals, primarily those in care homes, helping them remain independent and supported.
The service aims to deliver timely, proactive, and reactive care through our multidisciplinary team, who use their skills in non-medical prescribing, physical assessments, and clinical decision-making to effectively manage patient care.
They work closely with GP practices to support high-risk patients, developing personalised care plans that address specific needs, provide education on self-management, and coordinate care with other healthcare services.
The service supports over 38 care homes across Easington and Sedgefield who have a direct line to our team for advice and guidance.
